ZADDY connects men with independent, licensed healthcare providers. Clinical and telehealth services are delivered by those providers and facilitated through our telehealth partner, OpenLoop Health, Inc., and its affiliated medical groups. Protected health information held by those providers is also governed by the applicable Notice of Privacy Practices.
Information We Collect
We collect the following categories of information:
- Information you provide. Name, email address, phone number, shipping and billing address, date of birth, and account credentials when you create an account or place an order.
- Health information. Responses to medical intake questions, symptoms, medical history, medications, and other details you share so a licensed provider can evaluate whether treatment is appropriate for you.
- Payment information. Payment card or other payment details, processed securely by our third-party payment processors. We do not store full payment card numbers.
- Automatically collected information. Device type, browser, IP address, pages viewed, and similar usage data collected through cookies and comparable technologies.
How We Use Your Information
We use your information to:
- Connect you with licensed providers and facilitate your evaluation, treatment, and prescriptions where appropriate;
- Process orders, payments, subscriptions, and shipments;
- Provide customer support and respond to your requests;
- Send transactional messages about your account, orders, and care;

- Send promotional communications you may opt out of at any time; and

- Healthcare providers and our telehealth partner to evaluate, treat, and care for you;
- Partner pharmacies to fill and ship prescriptions issued by your provider;

- Authorities when required by law, regulation, subpoena, or to protect the rights, safety, and property of you, ZADDY, or others.
Your Health Information & HIPAA
When you receive care from a licensed provider through ZADDY, the provider and our telehealth partner act as your covered entity under the Health Insurance Portability and Accountability Act (HIPAA). You have the right to access and request corrections to your medical record, request confidential communications, ask us to limit certain uses and disclosures, receive an accounting of disclosures, and file a complaint without fear of retaliation.
